Transitional Housing Program plus Foster Care
THP-Plus Foster Care (THP+FC) is a licensed placement for youth, ages 18-21 who are participating in extended foster care made available by Assembly Bill 12. THP+FC is modeled after the original THP-Plus Program, and provides housing and comprehensive supportive services. THP+FC providers are certified by county departments of social services and licensed as Transitional Housing Placement Providers by the Community Care Licensing (CCL) Division of the California Department of Social Services.
Transitional Housing Program Plus
"The Transitional Housing Placement Plus (THP-Plus) program provides 24 months of housing, case management, and supportive services for youth aging out of the foster care system. Because of the complex barriers they face, former foster youth are at a higher risk of becoming homeless than their peers. THP-Plus provides the critical step in improving this transition to adulthood. THP-Plus participants receive assistance in developing an individualized budget and savings plan, setting educational and career goals, and building independent living skills. Youth also receive counseling and 24-hour crisis intervention, assistance with job readiness and mentoring to achieve goals of permanent housing and self sufficiency.
"Eligible youth between the ages of 18 and 24 years can be placed on the THP-Plus wait list, which is maintained by Santa Clara County’s Independent Living Program Unit. The county is responsible for determining eligibility and making referrals to the program."
